
Section Overview
The Section of Transition Medicine addresses the health care needs of older adolescents with developmental disabilities (IDD) who will require significant care coordination and case management in addition to ongoing medical care as they enter the adult health care system. This includes Autism, Cerebral Palsy, Spina Bifida, Down syndrome and additional genetic disorders for which pediatric care is no longer available.

Education
The section provides elective rotations for residents, medical students and nurses, and internship opportunities for social work students. We also offer a fellowship in Developmental Medicine.
Research
The Section of Transition Medicine focuses its research on improving methods of delivery of care and support for young adults transitioning out of pediatric care.

Our Transition Medicine Clinic is home to a strong group of healthcare providers that specialize in care for adults with developmental disabilities.
