Complete the online application below and submit all supporting materials no later than 5 p.m., May 15, 2020. Applicants are also responsible for making sure dean's certification and conduct check form, letters of recommendation, and unofficial transcripts are received by the deadline.
Please read the application requirements on this page before filling out the online application form. Unofficial transcripts can be uploaded to the online application and must include the fall grades. You may email your transcript to email@example.com if the fall grades are not complete at the time of completion of the application.
Online Application Form
It is important for applicants to be aware that the online application form must be completed in one session as there is no save feature. We recommend that you review a sample of the application form and prepare your responses in advance (and save them to your computer just in case). If your application is successfully submitted, you should receive confirmation via Web page.
Long Text Entry Fields: The application contains several open text fields to enter employment history, co-curricular involvement, research training/experience, and health equity/social justice experience. There is no word limit for these fields, but you are required to follow the format instructions.
Essay Questions: There are also two required essay questions and one optional essay. Applicants must adhere to the word limit and responses should be free from grammatical and spelling errors.
You need to upload your current transcripts and all other transcripts from the previous colleges you attended when completing the online application. You do not need to submit the previous college transcript if the current one has a complete letter grade from the previous college. These may be unofficial. However, you must present an official transcript if you are selected to participate in the program.
Dean's Certification and Conduct Check
In addition to the online application, each applicant must submit a completed Dean's Certification and Conduct Check Form confirming that you are in good institutional standing, without professionalism, student conduct or honor code concerns. This can typically be completed by someone in the Division of Student Affairs/Services on your campus.
At least (2) letters of recommendation are required. One letter must be from a faculty member at your school, preferably your prehealth/premedical advisor. The other(s) can be from academic or professional sources (letters from friends or relatives will not be accepted). Each of your references will need to complete the online evaluation form (https://www.jotform.com/build/83466138300150) by the March 15 deadline.
You will receive an email notification once a letter has been received on your behalf. Keep in mind you will not have access to the letters. Please convey the above requirements to your referees before submitting your completed application.

Each eligible applicant who submits a complete application package by the deadline will be offered a face-to-face interview in Houston by the COE’s TPP Admissions Committee to discuss their interest in the program and assess non-cognitive skills such as communication skills, maturity and potential for leadership as a physician.
NOTE: This application is specifically for consideration into the Center of Excellence in Health Equity, Training and Research Scholars Program. It is not intended to by an application for any other Baylor College of Medicine program, including admission to a degree granting program offered at Baylor.
